2012-2013 Application Information
At this time, we are accepting applications to attend one of our public school preschool rooms at Thomas Jefferson, Mark Twain, Neil Armstrong, Grant Wood and possibly Herbert Hoover. These rooms are first filled with students who qualify for free/reduced lunch. If space allows, non-free/reduced students are enrolled.

Our initial screening will begin with the following forms being completed (find these forms on the left):
  • Bettendorf Preschools Student Enrollment Form
  • 2012 – 2013 Preschool Eligibility Application
  • Home Language Survey
  • Preschool Transportation Form (if applicable)

In addition, we will need the following items on file to ensure your child qualifies for the program:
  • Photocopy of your child’s proof of birth
  • Photocopy of your child’s immunizations
  • Proof of residency (Proof of residency can be a current utility bill showing your name, address and the date, a lease agreement, or an approved offer to purchase or build a home.)

Please complete the four forms, copy the three additional items, and mail to the address below. If you prefer, all may be returned in person to 610 Holmes Street. Photocopies will be made for you if you bring the items in person.
